Returning to Sport After Injury: A Physiotherapist's Timeline
Pain-free doesn't mean ready. Strength, movement, and confidence under load are often still lacking.
Serious return-to-sport protocols rely on objective testing — 90% strength symmetry between limbs, hop tests — not how you feel. Returning too early is the number one cause of re-injury.
"It doesn't hurt anymore" is the most dangerous sentence a recovering athlete says to their physiotherapist — because it's usually true and insufficient at the same time. Pain typically resolves long before full strength and motor control return, and that exact gap is what produces most second injuries. Here's how physiotherapists actually decide — not by guessing and not by the calendar alone — when an athlete is genuinely ready.
Why pain is a poor readiness marker
Pain resolves once tissue has healed enough to tolerate ordinary daily movement. But returning to sport demands far more than that: full muscular strength, the capacity to tolerate repeated loading (jumping, rapid direction change, contact), precise motor control under fatigue, and genuine psychological confidence in the injured limb. All of these can still be lacking despite zero pain — which is exactly why "I feel fine" is an insufficient assessment on its own.
The four phases of a safe return
1. Restore movement and basic control
The first phase restores full range of motion and basic muscular control in the injured area. No sport-specific training yet — just rebuilding the basics: normal walking, stable single-leg standing, controlling the joint through its full range without compensating from other areas.
2. Build strength under progressive load
Progressive resistance training targeting the muscles around the injury and the full kinetic chain connected to it. Progress is measured in actual numbers — weight lifted, repetitions, limb symmetry index — not general feeling.
3. Reintroduce explosive and directional load
Jumping, landing, acceleration, change of direction — the movements that actually mirror the sport's real demands. This phase reveals deficits that don't show up in simple strength exercises, because it tests motor control under real speed and load.
4. Objective return-to-sport clearance
Before full return, the athlete is objectively tested: limb symmetry index (strength and performance on the injured side versus the uninjured side), hop tests (jump distance, landing stability), and movement quality screening under fatigue. Most serious protocols require at least 90% symmetry before clearing full training or competition. Returning at only 70-80% is well documented as a re-injury risk marker — and often a more severe injury than the first one.
Why rushing causes the second injury
The first weeks back carry the highest re-injury risk in most sports. The reason is direct: training load jumps rapidly after a period of reduced activity, and any lingering strength or motor control asymmetry — invisible during gentle rehab exercises — gets suddenly exposed under real competitive demands. This is precisely why objective clearance testing exists: to catch what the athlete feels is fine, but isn't fully ready yet.
